Connecting Practice to Data: Implementation Strategies to Increase Collection of Core Outcome Measures in an Inpatient Rehabilitation Facility
Capturing analyzable data about clinical care as a by-product of care is a foundational component of learning from practice in a learning health system. But capturing meaningful data that can be analyzed may require focused implementation efforts.
